FDMR Membership Form – Single Member

This form is used to renew your existing Single Membership or submit a new Membership application online and use a credit card to pay for it.

This is a TWO STEP PROCESS.

• Step One completes the membership form;
• Step Two completes the payment.

1. Enter your information in the form
2. Click Submit
   • You will see your completed form followed by a blank form (ignore the blank form)
3. SCROLL TO THE BOTTOM OF THE PAGE and click the PAY NOW button to complete your payment
   • Your registration won’t be complete until your credit card payment is submitted.

Step 2 – Complete Payment

Submitting the form does not complete your registration.
You must click PAY NOW below and complete your Stripe payment to finish.
